Female Hormone Replacement Therapy & Vaginal Estrogen Therapy Treatment from Dr. Steven R. Goldstein MD

Dr. Goldstein is a Professor of Obstetrics and Gynecology at New York University School of Medicine, Director of Gynecologic Ultrasound at NYU Langone Medical Center and the author of over seven books on Gynecology. He is regarded as one of the best Gynecologists in the US.

He is also a Certified Menopause Practitioner, North American Medical Society and a Past President of the North American Menopause Society. He is an expert on Female Hormone Replacement Therapy and co-author of the book “The Estrogen Alternative”. Dr. Goldstein has this to say about vaginal health.

There are two places where estrogen indefinitely is a good thing. And I find myself having to address these arenas separately. They are bone health and vaginal health. This article addresses vaginal health. There are very few species, like us, that outlive their reproductive capability. Estrogen deficiency of menopause results in vaginal atrophy and without treatment this simply gets worse over time.

There is a reduction in blood flow, elasticity, as well as PH change. Atrophy in the vagina can result in dryness, burning, painful intercourse, itching, as well as urinary issues like frequency, urgency, and a predisposition to urinary tract infection.

These symptoms as well as these changes can be significantly prevented or reversed with the use of local vaginal estrogens. There are currently three delivery systems - vaginal creams, small vaginal tablets of bio-identical estrogen, and an inert vaginal ring.

So if you are menopausal, and on no hormones, ask me about vaginal health at your next visit.

Dr. Steven R. Goldstein MD - Gynecologist in Manhattan, Views on Vitamin D

Dr. Steven R. Goldstein MD is an expert Gynecologist in Manhattan, New York in practice for over 25 years. Each patient receives individualized care and Dr. Goldstein is the only medical personnel you will see from the beginning to the end of your visit. Dr. Goldstein personally performs all examinations and transvaginal ultrasounds and Sonohysterography. Dr. Goldstein wrote the following regarding Vitamin D.

There is a difference between vitamin D deficiency (based on USRDA) that causes rickets in children and osteomalacia in adults and vitamin D insufficiency. A study showed 48% of women on medication for osteoporosis/osteopenia had insufficient vitamin D levels. Vitamin D is essential for normal absorption of calcium and adequate calcium is essential for bone health.

The main source of vitamin D is sunlight but diminished outdoor activity and the use of sunscreen has reduced that source. Dietary sources are very limited (milk, fortified cereal).

Blood measurement of vitamin D is expensive and not all that accurate. Thus I have been recommending patients supplement with 800 IU vitamin D per day. The 400 IU capsules are tiny, cheap and unlike calcium have no gastrointestinal side effects. If you take a multivitamin check the label as well as any calcium supplements - not all have Vitamin D and if they do the amounts are variable. Get some vitamin D supplements for yourselves and while you’re at it, give some to your daughters as well.
