Female Hormone Replacement Therapy

As women age and their ovarian function ceases (the medical definition of menopause), there is a lack of sufficient estrogen in the body. This results in real medical consequences for a woman’s body. Most women are very familiar with the menopause symptoms - hot flashes, vaginal dryness, foggy memory, night sweats, mood changes and so on which are very common during the menopausal transition. However, there are many more serious issues occurring in the body because of a lack of estrogen.

A serious consequence of a lack of estrogen is rapid bone loss which may result in osteoporotic fractures, in addition to joint pains. There is the risk of heart disease as LDL (bad) cholesterol and total cholesterol rises an average of 15-20% due to the lack of estrogen. Weight gain during menopause (fat accumulation around the midsection) is also due to a lack of estrogen in women. Changes in the vagina also occur because of a lack of estrogen, which causes a change in the normal bacteria thus affecting the pH of the vagina which results in dryness, a lack of lubrication and painful intercourse.

So what is the solution to this loss of estrogen in menopausal women? As an expert on Female Hormone Replacement Therapy, Dr. Goldstein prefers those approaches that combine the estrogen Premarin with a different category of drug than progesterone. That category is SERM (selective estrogen receptor modulator) used in Female Hormone Replacement Therapy. This synthetic estrogen is delivered via pills, patches or creams.

If you are a menopausal woman struggling to cope with the symptoms of menopause and concerned about the changes in your body, please schedule a consultation with Dr. Steven R. Goldstein MD to discuss the use of SERMs. Dr. Goldstein is an expert on menopause and Female Hormone Replacement Therapy. He is a Professor of Obstetrics and Gynecology, New York University School of Medicine and a Director of Gynecologic Ultrasound, NYU Langone Medical Center. Dr. Steven R. Goldstein MD, Gynecologist in Manhattan on - “Chemical” Pregnancies

Dr. Steven R. Goldstein MD, a top Gynecologist in Manhattan writes regarding the “Chemical” Pregnancies and miscarriages:

Gynecologist Manhattan

Regarding pregnancies, the home pregnancy test will turn positive at the time of the expected menses. Unfortunately, many of these will end up as “chemical pregnancies” and bleed before any ultrasound detection is possible.

The prognosis for such chemical pregnancies, however, is excellent going forward. Studies have shown that when there is such an occurrence, the rate of a healthy intrauterine pregnancy in the next month is 35% and, actually, reaches 83% within six months. This is an excellent prognostic sign. Such a patient has ovulated, has at least one open tube, has a partner who makes sperm and she is capable of capacitating his sperm.

Although such patients feel they have suffered a “miscarriage,” their prognosis is excellent going forward. Dr. Goldstein has not had a patient who miscarried in more than 25 years. He has just as many cases of pregnancy failure but because of early pregnancy monitoring with ultrasound, he has been able to diagnose virtually all of pregnancy failure prior to its spontaneous passage (i.e. miscarriage).

By using early ultrasound and monitoring a pregnancy to see that it is developing normally, when a pregnancy does fail, Dr. Goldstein can virtually always detect that before it is spontaneously passed. This prevents ending up in the Emergency Room at 4AM and needing an emergency D&C.

Dr. Goldstein, an expert Gynecologist in Manhattan, is a Professor of Obstetrics and Gynecology at New York University School of Medicine, the Director of Gynecologic Ultrasound at NYU Langone Medical Center, and is recognized as one of the nation’s best Gynecologists.
